Your privacy matters to us. When you trust us with information about your health, we have a responsibility to protect it.

Our privacy commitment

Fairview Mall RMT Clinic collects, uses, stores and shares personal and personal health information only as reasonably necessary to provide your care, operate the clinic and meet our professional and legal responsibilities.

Information We May Collect

Depending on the services you receive, we may collect information such as:

  • Date of birth
  • Health history
  • Current symptoms, concerns and treatment goals
  • Information provided on intake and consent forms
  • Appointment and treatment history
  • Clinical notes and assessment information
  • Insurance and direct-billing information

We try to collect only the information reasonably necessary for your care and our legitimate clinic responsibilities.

  • Meet professional, regulatory, insurance, accounting and legal requirements

Sharing Your Information

We do not sell your personal health information.

Your information may be shared only when reasonably necessary and permitted by law, including with:

  • Your treating healthcare professional
  • Authorized clinic staff
  • Insurance companies or benefit providers when authorized or legally permitted
  • Technology and service providers that help us operate our clinic and booking systems
  • Professional advisors or regulatory authorities when legally required
  • Another healthcare provider when you authorize the disclosure or when otherwise permitted by law

Where specific authorization is required to disclose personal health information to a third party, we will obtain it.

Appointment Email & SMS Messages

If you provide us with an email address or mobile phone number, we may use it for important service-related communications, such as:

  • Appointment confirmations
  • Appointment reminders
  • Schedule changes
  • Intake-form reminders
  • Information directly related to your treatment, account or clinic visit

These operational messages are different from promotional marketing communications.

Optional Newsletters, Health Tips & Promotions

From time to time, we may offer patients the option to receive clinic news, health information, service announcements and promotions by email or SMS.

  • Where consent is required, marketing consent is optional.
  • You may unsubscribe from promotional communications at any time.
  • Choosing not to receive marketing will not affect your ability to receive treatment.

Protecting Your Information

We use reasonable safeguards to protect personal information against:

  • Loss or theft
  • Unauthorized access
  • Unauthorized use or disclosure
  • Improper copying or modification
  • Improper disposal

Access to patient information is limited to individuals who reasonably need it to perform their responsibilities.

Keeping Your Health Records

Patient health records are retained for the period required by applicable Ontario laws and professional standards.

For Registered Massage Therapy records, this generally means 10 years after the patient’s last visit, or, where the patient was under 18 at the last visit, for the required period following the patient’s 18th birthday.

Records are disposed of securely when they are no longer required.

Your Privacy Rights

Subject to applicable law, you may:

  • Ask to access your personal health information
  • Ask us to correct factual information that is inaccurate or incomplete
  • Ask questions about how your information is handled
  • Withdraw or limit certain consent where legally permitted
  • Raise a concern or make a privacy complaint

Some information must still be kept or used when required by law or professional standards, even after consent has been withdrawn.

Policy Updates

We may review and update this Privacy Policy from time to time as our clinic, technology, services, professional requirements or applicable laws change.

Changes may be made without individual advance notice. The most current version posted on our website or patient booking system will apply from its stated effective date.

Nothing in a policy update removes privacy rights or protections provided to you by applicable law.
