Your appointment time is reserved specifically for you. Once your booking is confirmed, the therapist may turn away other appointment requests for that time.
We understand that illness, family emergencies, work situations and other unexpected events can happen. We simply ask patients to provide as much notice as possible when plans change.
1- Your Appointment Is Reserved for You
When I book an appointment, I understand that the therapist reserves that time specifically for me.
The therapist may turn away other appointment requests once my time has been reserved.
A last-minute cancellation or missed appointment can result in lost income for the therapist and an appointment that another patient could have used.
2- 12-Hour Cancellation Notice
I agree to provide at least 12 hours’ notice if I need to cancel or change my appointment.
Appointments cancelled with less than 12 hours’ notice may be subject to a fee of up to 50% of the scheduled treatment fee, at the clinic’s discretion.
The clinic understands that genuine emergencies happen. Exceptional circumstances may be reviewed individually and a cancellation fee may be reduced or waived at the clinic’s discretion.
3- Missed Appointments / No-Shows
If I do not attend my scheduled appointment and do not provide advance notice, I understand that a missed-appointment fee may apply.
For appointments where significant time has been specially reserved, including certain:
- New-patient appointments
- Couples or multiple-person appointments
- Extended appointments
- Special bookings
A fee of up to 100% of the scheduled appointment fee may apply at the clinic’s discretion. Reasonable circumstances will be considered before a fee is applied.
4- Same-Day Appointments
When I make an appointment on the same day, I understand that the therapist is reserving that time specifically for me.
Where possible, I agree to give at least 4 hours’ notice if I need to cancel a same-day appointment.
A late-cancellation fee may apply when adequate notice is not provided.
5- Arriving Late
If I am running late, I will try to contact the clinic.
The therapist will make reasonable efforts to accommodate me using the appointment time remaining. However, the appointment normally cannot be extended if another patient is scheduled afterward.
My treatment may therefore be shortened and the original booked appointment fee may still apply.
6- Appointment Communications
By providing my telephone number and email address, I understand that Fairview Mall RMT Clinic may contact me for communications reasonably connected with my healthcare services or account, including:
- Appointment confirmations
- Intake-form reminders
- Clinic location or arrival instructions
- Billing or account matters
- Appointment reminders
- Changes to appointment times
- Information relating to my treatment
These service-related communications are separate from optional promotional marketing.
Policy Changes
Fairview Mall RMT Clinic may update or change its policies from time to time to reflect changes in clinic operations, services, technology, professional requirements or applicable laws.
Policies may be updated from time to time without individual advance notice. The current version posted on our website, patient portal, booking platform or otherwise made available to patients will apply from its stated effective date.
Any change remains subject to applicable laws, professional standards and patient rights.
